NBA Jam Extreme, developed by Sculptured Software and published by Acclaim Entertainment, was the first NBA Jam title under the Acclaim brand and the first in the series to use 3D graphics. The cover features Seattle SuperSonics forward Shawn Kemp and Houston Rockets centre Hakeem Olajuwon, with some high-profile names absent due to competing sponsorship deals. This UK Original release carries serial SLES-00529 and was released on 29 November 1996. It is a single-disc Full Jewel PAL release in English, with no hologram on the case. As the sole catalogued PAL variant, this UK-only pressing is the only documented physical release for the title across the PAL region.
